(Belfast, ME) On behalf of the entire staff, we would like to welcome you to Seaport Family Practice. Since 1984, we have offered a team approach to medical care for the whole family. We are especially honored to care for patients at the beginning and end of life, and to care for multiple generations. Our success depends upon the trust and friendship we build into our relationships. We look forward to meeting and working with you.

In 2006, Seaport Family Practice was invited to participate in the National Demonstration Project, a two-year study undertaken by the American Academy of Family Physicians on practice transformation. We were one of 36 practices chosen nationwide, and the only practice in Maine. Over the last 3 years, we have adopted many of the principles of the Patient Centered Medical Home, and now qualify for NCQA certification. In 2010, we joined two dozen other primary care practices in Maine's Pilot Study of the Patient Centered Medical Home. Change is difficult for everyone, not the least for you, our patients. The ultimate goal is to improve the care we provide for you.

This is our pledge: We will try to help you meet most of your health care needs on the day they arise. We offer Open Scheduling, so that most of our appointments are available at the start of each day. If all appointments are full by the time you call, we will try to answer your questions over the telephone, direct you to the emergency department, or "work you in" with one of the day's providers- whichever best suits the need. We try to schedule regular appointments on the day the patient calls. However, if you need to schedule an appointment in advance, we will your individual needs into account. You can request an office appointment or annual physical exam online through Webview.  A physician is always available by phone (338-6900) After-Hours and on weekends; often we can arrange to meet you at the office.

 

We accept: Medicare, Medicaid, Blue Cross/Blue Shield & most managed care plans. We also offer a Sliding Scale fee schedule to qualified applicants.

Notice to Patients:

On Wednesday February 8, SFP will be doing a computer software upgrade.  The office WILL be open, however, there will be NO access to computers or webview for the whole day.  Appointments that have been scheduled will have printed documents appropriate for your visit that day.
We are in hopes all will go smoothly and be back on track Thursday a.m.
